Registering with the practice

We have an open list and welcome requests for registration from patients living or moving into the Practice area. We can only register people living within the Practice catchment area.

You can register by asking for a form at Reception or alternatively send a request with your name and address here, and  we can post the necessary forms out to you.

If you are aged 16 or over we will need to see two forms of identification, one must include a photo and one must include your registering address. For children under the age of 16, we will need to see a birth certificate or passport.

You will be allocated a GP at the Practice who will lead on your care, but you are not restricted to just seeing this GP. Please allow 48 hours for our records to be updated with your details prior to trying to make an appointment or request any medications.

We recommend that all new patients over the age of 16 undertake a 20 minute Health Check with one of our Healthcare Assistants.

If you are homeless, you are still entitled to register with a GP using a temporary address, which may be a friends address or a day centre. Further advice is available here

You can register by completing a registration form either obtained from the practice reception or downloadable via the link below.

Completed registrations should be printed and handed in to reception.


Before completing the registration form, please review whether your address falls into our practice area. We are unable to accept patients who live outside the boundary. If you are unsure, please contact the surgery and speak to reception.

Please ensure the GMS1 form and registration document is completed in full. The GMS1 form filled properly will help to speed up the process of registration and getting your records from your previous practice.

The practice area includes Gosberton and its surrounding areas.  A map of the practice area is available to view at the surgery

If you are registering with us and we will be your first GP in the UK and you do not have an NHS number there is few bits of extra information we will require to process your registration:

  • Information regarding place of birth and date of entry to the UK on GMS1 form
  • Patient declaration on back of GMS1 form
  • It would also help the practice if you could provide any copies of vaccinations or medications you have previously been on

Visitors to the area who need medical help or advice are welcome at our practice.

You may have recently left the forces and wish to register with us. You will need to live within the practice boundary or already live with a family who are registered with us. When you leave the forces, you should be given the option of receiving your medical records for you to pass on to the GP Practice. Alternatively, you will be asked to complete the Military Medical Record Request Form (see below link), which should be handed in when you register with us for us to request your records from the military.

Requests for military medical records can take up to 6 months for us to receive your notes so we would advise that you get copies of your records before you leave so we can merge these with your NHS records.
