Medication After Hospital Discharge

When you are discharged from hospital, if required, you should normally receive a supply of medication.

The hospital will provide you with a list of your medication requirements (discharge documents) when you leave. Please arrange for this to be handed in to the surgery well before your supply of medication runs out.

Any requests from the hospital to change your medication will be checked by a GP first. If necessary, your doctor will then issue a prescription. The Practice will aim to provide your prescription within three working days.

Your medication will be reviewed regularly by the doctors, and this may involve changes to your tablets in line with current policies.
