Patient Notice

If you are attending the surgery for a face-to-face appointment and have any symptoms of COVID-19 (such as cough, fever, sore throat, or cold-like symptoms), we kindly ask that you wear a face mask while in the building.
This is to help protect other patients, especially those who may be more vulnerable to infection.

Sick Notes

We can only issue a sick note if you’ve been off work for 7 days or more.
If you need a new or continued sick note, please complete “I have an admin or routine care request” on our patient triage service (Monday - Friday 7:30am–6:30pm). Your note will be sent by email or text.

Self Certification

If you’ve been unwell for less than 7 days, you’ll need to self-certify using this form: What is your name? - Personal details - Ask your employer for Statutory Sick Pay - GOV.UK
